The book has pages numbered 1 through 109, plus i through xvii for the up-front stuff. Oh, and four unnumbered pages of pictures.

The index is pages 104 through 109.

What he calls 'references' are standard citations of books and other literature. They appear on pages 91 through 103. He cites them throughout the text, but in a format which is unusual to my eye. I did not cross-check to verify that every item listed tracks back to a point in the text and vice versa.

I fear that my effort to provide what you request would result in a reproduction of about half of the text, and half of the references. So here is the publisher's dust jacket blurb:

Kindleberger sets out to challenge the widespread belief that the market for seafarers, in the days before steam, was efficient - conforming more or less to a strong prior belief in the neo-classical economic model of supply and demand.
The historical literature on the subject is traditionally strewn with references to crimping or shanghaiing, naval press-gangs, desertion, mutiny, marooning and shipwrecks due to drunkenness or negligence. Kindleberger looks at the issues of recruitment and pay, the treatment of seamen ashore and afloat, the question of government intervention and its impact on efficiency.

To which I add that he says plenty about navies because of the impact of wars on employment. Also:

The two main 'nurseries' or 'cradles' or institutions for 'breeding' seamen in Britian were deep-sea fishing and the collier fleet carrying coal from Newcastle to London that went back to Tudor times.
